Social Worker The purpose of this position is to screen and assess referrals, develop case plans and related documentation, and to provide clients with appropriate supportive services. Performs intake duties, provides care assessments plan, and coordinates and monitors services. Provides periodic case reviews. Completes court reports and provides testimony. Maintains contacts with other social service personnel and service providers; makes referrals to other services as appropriate. Maintains contacts with clients and family. Maintains files, answers telephones, and performs other general office work as required. Attends various staff training, and other meetings. A completed application and resume are required.
